Here’s what the research actually says about vitamin D benefits for pain and energy, the three ways to optimize your levels that matter most, and where vitamin D fits in a broader natural pain relief strategy.
Vitamin D Benefits: What’s Real and What’s Overhyped
If you’ve been dealing with persistent aches, low energy, or that heavy, foggy feeling that doesn’t resolve with sleep, someone has probably told you to check your vitamin D. They’re not wrong to suggest it — but the full story is more interesting than “take a supplement and feel better.”
Vitamin D isn’t really a vitamin in the classic sense.
It functions as a hormone, with receptors in muscle tissue, immune cells, the nervous system, and nearly every organ in the body. That’s why deficiency can show up in so many different ways: musculoskeletal aches, fatigue, low mood, frequent illness, slow recovery from exercise.
Here in San Jose, you might assume sunshine takes care of it. In practice, we see deficiency regularly — and it makes sense. Most of us work indoors, commute in cars, wear sunscreen daily (as we should), and spend the brightest hours of the day at a desk. Living in a sunny climate doesn’t mean your skin is making vitamin D.
Vitamin D and pain: an honest look at the evidence
The research here is genuinely mixed, and it’s worth being clear about what it shows.
On the association side, a large UK study of nearly 350,000 adults found that severe vitamin D deficiency is associated with chronic widespread pain — the kind that shows up in multiple areas rather than one spot. Milder deficiency showed weaker associations once lifestyle factors were accounted for.
On the treatment side, results depend heavily on who is being studied. A meta-analysis of 19 randomized controlled trials with over 3,400 participants found significantly greater pain reduction with vitamin D supplementation compared to placebo across chronic pain conditions — with the clearest effects in people who were deficient to begin with. Other analyses, particularly for chronic low back pain in people with adequate levels, show little to no benefit.
The takeaway is consistent with what we see clinically: correcting a real deficiency can meaningfully reduce pain, but vitamin D is not a painkiller. If your levels are already adequate, more vitamin D won’t do more. If they’re low, getting them into a healthy range removes a quiet contributor to muscle aches, slow recovery, and amplified pain signaling — and makes everything else you’re doing for your pain work better.
Vitamin D and energy
A similar pattern holds for fatigue. Randomized controlled trials in people with confirmed deficiency — including a well-designed double-blind trial of vitamin D3 for self-perceived fatigue — show measurable improvements in energy after supplementation. In people with normal levels, large genetic studies suggest vitamin D status isn’t a meaningful driver of tiredness.
So the question isn’t “should everyone take vitamin D for energy?” It’s “is low vitamin D one of the things draining yours?” For a significant share of indoor-working adults, the answer is yes.
3 Ways to Optimize Your Vitamin D
1. Get smart, consistent sun exposure
Your skin produces vitamin D when UVB rays hit it — but only under the right conditions. UVB is strongest between roughly 10am and 3pm, and the amount you synthesize depends on skin tone, age, and how much skin is exposed.
What works in practice:
- 10–20 minutes of midday sun several times a week, with forearms and lower legs exposed, is enough for many lighter-skinned people in our latitude
- Darker skin tones need meaningfully more time — melanin is natural sun protection, which also slows vitamin D synthesis
- Adults over 60 produce less vitamin D from the same exposure, so sun alone is rarely sufficient
- Sunlight through a car or office window doesn’t count — glass blocks UVB
This is not an argument against sunscreen. For exposure beyond those short windows, protect your skin. The goal is brief, regular, intentional exposure — a walk after lunch does double duty for vitamin D and for the movement that stiff, achy bodies need anyway.
2. Eat for vitamin D — and for absorption
Food alone rarely fixes a deficiency, but it provides a steady baseline and supports everything else:
- Fatty fish — salmon, mackerel, sardines (2–3 servings a week is the most efficient dietary source)
- Egg yolks, especially from pasture-raised hens
- UV-exposed mushrooms — one of the few plant sources
- Fortified foods — milk, plant milks, and some yogurts
Just as important: vitamin D is fat-soluble, so it absorbs best with a meal containing fat — olive oil, avocado, nuts, fish. This applies to supplements too. A vitamin D capsule swallowed with black coffee on an empty stomach is a very expensive way to change very little.
